• Company IDs issued by Private Entities or Institutions Registered With/ Supervised/Regulated by the Bangko Sentral ng Pilipinas (BSP), Securities and Exchange Commission (SEC) or Insurance Commission (IC)
Submit the following documents certified as True Copy by your Corporate Secretary:
• SEC Certificate of Registration/Incorporation or DTI Certificate of Registration (for Sole Proprietorship)
• Articles of Incorporation/Partnership (or its equivalent), and By-Laws
• Latest General Information Sheet (GIS) filed with the Securities and Exchange Commission (SEC)
